This report gives total paid claims sorted by:

Information Needed: 

• Group Name
• Date Range
• Sort Type and Special Requests (i.e. sort by division, individual, or benefit type)

Charge Paid Detail Report

This report gives total claims by individual and includes the amount charged, the PPO savings, the amount ineligible, the deductible, the out-of-pocket and the amount paid. It can be sorted in the following ways:

• By case, which lists total claims by individual (sorted alphabetically)
• By division, which lists total claims by individual (sorted alphabetically) by division
• By case by charge type, which lists total claims by charge type by individual (can also be broken down further by division)

The Charge Paid Detail Report can give one total per individual or can print each individual claim.

• Group Name
• Date Range
• Sort Type and Special Requests (i.e. sort by division, individual, or benefit type)

Claim Analysis

This report includes a breakdown of total claims by charge type and claim lag study. This report can be broken down by the entire case or division.

Information Needed:

• Group Name
• Date Range
• Sort Type and Special Requests

Claims Excess Loss Report

This report lists total claims for all individuals over a specified dollar range. This report will include all claims, weather or not they are covered under the specified contract. The company name is usually setup as an individual where administrative fees and prescription drug card invoices are processed.

Information Needed:

• Group Name
• Date Range
• Sort Type and Special Requests

Contract Savings Report

This report gives the original PPO charge amount, the adjusted PPO charge amount and the PPO savings. It can be sorted and broken down in the following ways:

• Sorted and broken by contract table
• Sorted by contract table and broken by division
• Sorted by division and broken by contract table

Information Needed:

• Group Name
• Date Range
• Sort Type and Special Requests

Payment History Report

This report lists the checks printed for a specified time frame. It can be sorted in the following ways:

• Alphabetically by individual
• By draft number (subtotaled by print date)
• By division
